If calories are kept within the range mentioned above, and carbohydrates are limited to whole food low glycemic carbs, limited to a net carb intake of 20-30 grams per day, fat loss should occur while on HGH replacement therapy.

I would rather be a very obese man. As the longest fast in history recorded was 362 days. If you need to live 362 days without food and just water, your genes will be better off at surviving to the next generation by being really obese.
The evidence currently health and longevity appear to point to most of us entering into a state of nutritional-ketosis cyclically. How often? That really depends on the person, and it is why you should work with a doctor knowledgeable in the nutritional modalities outlined above.
As mentioned above, there are extreme cases and everything in between. A normal distribution curve or bell curve of all shapes, sizes, and conditions. The good news is we have more tools in our tool chest than ever before.
